GLB-AIM (Adapted for Impaired Mobility) DPP is a 12-month virtual lifestyle change program based on the CDC’s Diabetes Prevention Program and the Group Lifestyle Balance curriculum, tailored for adults with mobility limitations. You’ll experience a full year of evidence-based support to build healthy habits, improve nutrition, increase physical activity safely, and reduce your risk of type 2 diabetes.

Guided by trained Lifestyle Coaches and supported by a welcoming online group, you’ll move through structured phases that focus on sustainable lifestyle changes, accountability, and confidence-building at your own pace.

What to Expect in the GLB-AIM Diabetes Prevention Program:

You’ll move through three phases of the program:

  • Core Phase: Weekly group sessions that introduce the foundational skills for healthy eating, physical activity, and behavior change.
  • Transition Phase: Sessions shift to bi-weekly as you learn to apply these skills more independently.
  • Maintenance Phase: Monthly touchpoints help you sustain your progress and stay accountable.

What’s the time commitment?

Weekly 60-minute sessions for the first 3 months, followed by biweekly and monthly follow-up sessions for ongoing support.

How do I sign-up for NCHPAD GLB-AIM DPP?

How much does the program cost?

The program is completely free! We only ask for commitment, dedication, and a good attitude.

Is this a weight loss program?

No. While some participants may experience weight loss as a result of lifestyle changes, the primary focus of DPP GLB-AIM is on improving overall health and reducing the risk of type 2 diabetes.

Do I need any special equipment?

No, all required materials and resources will be provided to you!

Do I have to be diagnosed with pre-diabetes to qualify?

Nope! This is a lifestyle program offered to those within the disability community.
