NCHPAD is a public health practice center providing inclusive health
promotion programs and resources for people with disabilities. NCHPAD is
funded by the Centers for Disease Control and Prevention (CDC),
National Center on Birth Defects and Developmental Disabilities
(NCBDDD), Division of Human Development and Disability, Disability and
Health Promotion Branch, Grant #NU27DD000022.

In 2022, we created NCHPAD Connect to collect and understand the needs, interests and lifestyles of people with mobility limitations across the United States.
Within this private wellness database, we use the data to create,
evaluate and enhance health promotion programs, develop educational
resources and improve inclusion. Individuals who join and contribute
their information to NCHPAD Connect have access to our free
evidence-based health and wellness programs, resources and virtual
community events, with additional opportunities to participate in and
further the development of new resources.

Eligibility Criteria:
• Live in the United States or its territories
• Be 18 years or older
• Self-report a mobility limitation or physical disability, or use a mobility device or assistive technology
No medical diagnoses or paperwork required

Fill Out the Interest Form: If eligible, you’ll be directed to our Interest Form to share more about yourself and your interests in our programs and services.

Complete the NCHPAD Questionnaire: This survey finalizes your enrollment in NCHPAD Connect. Your responses help us understand your health screening experiences, healthcare access, lifestyle behaviors, health conditions, vaccination status and life satisfaction. Your answers are crucial for creating and refining meaningful programs and resources for people with disabilities.

We use the information provided by our participants to:

Evaluate and Enhance Programs: We continuously improve our programs based on participant feedback and data.

Publish Findings and Educate: We use the data to create educational materials and share insights about the experiences of individuals with physical disabilities or mobility limitations. We also public scientific papers to inform the research community about the insights and discoveries of health promotion and wellness in people with disabilities. Nevertheless, personal identifiable information such as names, email addresses and phone numbers are NEVER shared.

Inform Decision Makers: We aim to improve access and inform policymakers and other decision makers to create better systems for the public.

Privacy Assurance: We do not sell, trade or rent your personal identification information. Any shared information is generic and aggregated, never linked to personal identifiers, and only shared within NCHPAD and our affiliates.
