The National Institute for Health Care Management Foundation released a new infographic entitled "Addressing Social Determinants of Health Can Improve Community Health & Reduce Costs." The infographic was developed from the Robert Wood Johnson Foundation County Health Rankings Model and the McKinsey 2019 Consumer Social Determinants of Health Survey. It quantifies the significant role social determinants play in an individual's health vs health behaviors and access to care alone. More specifically, it highlights different kinds of unmet social needs that surveyed communities reported, shows how specific determinants such as transportation affect access to emergency care, and various solutions being implemented across the country to address unmet needs.
