Program Information


Prevent T2 for All is an adaptation of  “Prevent T2,” the 12-month lifestyle change program to prevent or delay Type 2 Diabetes administered by the Centers for Disease Control and Prevention (CDC) Division of Diabetes Translation. This inclusive program is designed to promote modest weight loss, increased physical activity, and other lifestyle changes in adults diagnosed with pre-diabetes.

The Lakeshore Foundation is partnering with the National Association of Chronic Disease Directors and Provention Foundation to train Lifestyle Coaches on the Prevent T2 for All curriculum.  For more information on these workshops, contact Mari Brick at mBrick_ic@ChronicDisease.org or go online to DPP Coach Training | provention (proventionhealth.org).  NCHPAD also provides inclusion workshops to trainers of chronic disease self-management, prevention, and other simliar programs.  For more information about these and all of NCHPAD's work in diabetes prevention, please contact Chris Mackey at chrism@lakeshore.org.
