Abbreviated Accessibility Checklist


This checklist is designed to highlight potential barriers to access to medical care for people with disabilities. Please note that a “Yes” answer does not guarantee that accessibility has been adequately addressed, but this survey will help you to develop plans for improved access. If a potential barrier is detected, please refer to Removing Barriers: Planning Meetings That Are Accessible to All Participants or the ADA resources listed at the end of the checklist for more detailed information on barrier removal.
