Inclusive Health Coalition Reach

There are currently 20 inclusive health coalitions (IHCs) operating nationwide to promote the inclusion of people with disability in community health transformation efforts. These coalitions are creating healthy opportunities for people with disabilities by establishing partnerships with other community health organizations, creating new, healthy, accessible, programs, and ensuring that inclusion is a cornerstone in community health planning.

IHCs are located in the following communities, providing them the opportunity to reach over five million people! Just click on the image to see the full-sized list of communities.

Click on the map below for the full-sized version to see if there is an IHC near you. If you are interested in getting involved, contact NCHPAD and we will put you in contact with your local IHC. Don't see your community on the map? Let us know, and we will try and provide you with guidance and informational resources to establish your own local inclusive health coalition.
