Two paid graduate-level fellowships to work as part of the Health and Disparities Project. This funding opportunity for local health departments (LHDs) to develop the capacity to deliver effective Stanford model of Chronic Disease Self Management Programs or Arthritis Self-management Programs, English or Spanish version, from here on referred to as CDSMP collectively. In expanding the availability of the CDSMP to additional local jurisdictions, we seek to expand the use of evidence-based programs while enhancing the well-being and self-efficacy of persons with a chronic illness.
