NHS Human Services, Pennsylvania's largest provider of human services, has an opening in our Therapeutic Family Care program for a Bundled Program Specialist. Responsibilities include, but are not limited to: provide weekly individual therapy for a caseload of 10-12 children; provide bi-weekly meetings and treatment with the child\\\'s legal family; create treatment plans for assigned caseload; facilitate Individual Service Plan meetings with the treatment team; Provide ongoing training and consultation with foster parents; participate int he completion reauthorization requirements for Managed Care Organizations; Facilitate monthly team meetings; Participate in On-Call coverage rotation; Maintain accurate documentation for licensing; adhere to NHS policies and regulations.
