Position Summary: Youth Counselor/Case Manager will be responsible for conducting intake and assessment for Youth Transition Assistance Project clients. S/he will initiate contacts with schools and develop individual Transition Action Plan with customers, schools, and families. Assess clients support needs and refer them to AHS in-house services and/or external resources. Provide support for vocational rehabilitation services. Youth Counselor/Case Manager will be under supervision of Program Director, Community Health Programs, and work collaboratively with community health team and other AHS staff.

Qualification and Requirements: Bachelor degree in social services/counseling or other health related field. Knowledge of MS Office, excellent customer service skills, experience in serving clients with disabilities and youth, familiar with transition service resources and disability benefits are preferred, ability to multitask, set priorities and meet deadlines, able to speak one of major Asian languages. Need to have a car with valid auto insurance.
