The National Center on Health Physical Activity and Disability (NCHPAD) engages in a number of partnerships both nationally and locally. Over the past two years, the health care sector has had a partnership with the Foundation for Physical Medicine and Rehabilitation (http://foundationforpmr.org/). The Foundation for Physical Medicine and Rehabilitation’s mission is to enhance health, function and quality of life for individuals with disability through education and research in the field of physical medicine and rehabilitation. This partnership has leveraged a number of activities and resources that ultimately benefit individuals with and without disabilities over the past two years including the following:
Rx for Exercise app/website
The National Center on Physical Activity and Disability (NCHPAD) and the Foundation for Physical Medicine and Rehabilitation (Foundation for PM&R) have created an app for health care providers to prescribe physical activity to their patients. The app is available for Apple and Android phones. It includes information on the following:

Rx for Exercise prescription pads
NCHPAD and the Foundation for PM&R have created Rx for Exercise prescription pads as an additional tool for the Rx for Exercise program. Free prescription pads can be ordered at the following website: http://foundationforpmr.org/rx-prescription-pad/
Rx for Exercise pediatric website
During the 2017-2018 year, NCHPAD and the Foundation for PM&R created the Rx for Exercise pediatric website. This website complements the Rx for Exercise program by adding additional resources specific to pediatric settings. It provides additional webinars, current education and research for prescribing exercise for children with disabilities, and information for patients. http://foundationforpmr.org/old/rx-for-exercise-pediatrics-new/
Additionally this year, we plan on creating a resource for hospitals to prevent post-rehab decline in patients. We look forward to our continued partnership as well as engaging in additional effective partnerships that leverage increased inclusion in all communities.
