Funding Opportunity Announcement
An Innovative Approach to Inclusive Health
The National Center on Health, Physical Activity and Disability (NCHPAD) seeks applications for a program planning and implementation project adapted for people with mobility, intellectual and/or sensory disability that increases access to physical activity, healthful food, and/or healthy weight management initiatives in an inclusive setting. The purpose of this Funding Opportunity Announcement (FOA) is to determine if programmatic, policy, systems and environmental (PPSE) changes can occur through an inclusive community implementation process. This application is open and available to the state organizations funded under the CDC-RFA-DD16-1603 “Improving the Health of People with Mobility Limitations and Intellectual Disabilities through State-based Public Health Programs. Of the 19 awardees funded under this Funding Opportunity Announcement (FOA), only the 14 awardees funded to implement Core and Enhanced Implementation activities are eligible to apply for this award. Applications must be submitted by the State Disability and Health Grantee or a Bona Fide Agent, which is an agency/organization identified by the state as eligible to submit an application under the state eligibility in lieu of a state application.
