The service delivery model described in this paper includes a Supportive Transitional Setting that offers people with disabilities the opportunity to learn to manage their own health and the accommodating secondary conditions that are associated with their disability. The primary goal is to empower people with disabilities to manage their own health by preparing them to access community-based programs armed with the necessary training and information to overcome environmental barriers to participation.
