There will be six (6) expected Awards with a ceiling of $800,000 each. To improve the quality of acute stroke care and health outcomes for acute stroke patients. Program activities will address the continuum of care from the onset of stroke through rehabilitation and recovery, focusing on health systems change and community and clinical linkages.
